Joseph Carroll Pilot Crash: Amazon Cargo Plane Pilot Injured After Miami Runway Overrun as Federal Investigation Continues

MIAMI, Fla. — Joseph Carroll, one of the pilots aboard an Amazon Air cargo flight involved in a catastrophic runway overrun at Miami International Airport, survived the crash and was treated for injuries as federal investigators continue working to determine what caused the accident.

The Boeing 767 cargo aircraft, operated by 21 Air on behalf of Amazon Air, was arriving from San Juan, Puerto Rico, on September 6, 2026, when it overran the runway during landing. The aircraft struck several vehicles on airport property before crashing through a perimeter fence and coming to rest off the airfield.

Carroll and the aircraft’s co-pilot were rescued from the damaged cockpit by emergency responders and transported to a local hospital for treatment. Authorities later confirmed that both pilots had been treated and released. Tragically, five ground workers traveling in a van struck by the aircraft lost their lives, while several others sustained injuries, including two people who remained in critical condition.

The National Transportation Safety Board (NTSB) has launched a comprehensive investigation into the accident. Investigators are examining numerous factors, including weather conditions, the aircraft’s landing speed, crew actions, runway conditions, and the absence of an Engineered Materials Arresting System (EMAS) at the end of the runway. Flight data and cockpit voice recorders have been recovered for analysis.

Amazon, 21 Air, Boeing, and federal aviation authorities have pledged full cooperation with the investigation. While preliminary information has been released, officials have emphasized that it is too early to determine the probable cause of the runway overrun.
